The Chiesa di Sant'Antonio di Padova is a seventeeth century church in Casamicciola. It's located just off the main road between Ischia Porto and Casamicciola.
To reach the church from Ischia Porto, take the main road west and stay on it as it passes Castiglione Thermal Park on the right before going down a hill and eventually bearing right towards the sea. When the sea comes into view again you will see that the road bends left at a corner between the Hotel La Madonnina and Albergo Terme Italia. At this point, turn right up the narrow road next to La Madonnina and you will shortly reach the church in a small square.
